Health Examination & Immunization Requirements

A student’s parent(s)/guardian(s) shall present proof that the student received a health examination, with proof of the immunizations against, and screenings for, preventable communicable diseases, as required by the Illinois Department of Public Health, within one year prior to:

  • Entering kindergarten or the first grade;

  • Entering the sixth grade; and

  • Enrolling in an Illinois school, regardless of the student’s grade (including nursery school, special education, Head Start programs operated by elementary or secondary schools, and students transferring into Illinois from out-of-state or out-of-country).

Exclusion

Unless an exemption or extension applies, the failure to comply with the above immunization requirements by October 15 of the current school year will result in the student’s exclusion from school until the required health forms are presented to the District. New students who register after October 15 of the current school year shall have 30 days following registration to comply with the health examination and immunization regulations. If a medical reason prevents a student from receiving a required immunization by October 15, the student must present, by October 15, an immunization schedule and a statement of the medical reasons causing the delay. The schedule and statement of medical reasons must be signed by the physician, advanced practice nurse, physician assistant, or local health department responsible for administering the immunizations.

HEALTH REQUIREMENTS FOR ADMITTANCE TO SCHOOL

KINDERGARTEN OR ENTERING SCHOOL FOR THE FIRST TIME

Physical Examination

  • Dated within one year prior to the first day of the school year

  • Health history portion of physical examination form must be completed and signed by a parent/guardian

  • Documentation of diabetes screening

  • Documentation of lead screening

 Immunizations

  • Diphtheria, pertussis, and tetanus (4 or more doses of DTP/DTaP with the last dose being a booster and received on or after the 4th birthday)

  • Polio (4 or more doses of the same type of Polio vaccine (IPV, OPV) with the last dose being a booster and received on or after the 4th birthday.

  • Measles, Mumps, Rubella (2 doses of each with the 1st dose being given on or after the 1stbirthday and the second dose no less than 4 weeks (28 days) later.  Usually given as a MMR

  • Varicella (2 doses of varicella vaccine with the 1st dose being given on or after the 1stbirthday and the second dose no less than 4 weeks (28 days) later, or statement from the physician or health care provider verifying disease history or laboratory evidence of varicella immunity

GRADE SIX

Physical Examination

  • Dated within one year prior to the first day of the school year

  • Documentation of diabetes screening

  • Health history portion of physical examination form must be completed and signed by a parent/guardian

Immunizations

  • Diphtheria, pertussis, and tetanus (3 or more doses of DTP/DTaP or Td with the last dose being a booster and having been received on or after the 4th birthday)

  • Tetanus, diphtheria, acellular pertussis (Tdap) (one dose regardless of the interval since the last DTaP, DT or Td dose)

  • Polio (3 or more doses of Polio with the last dose being a booster and having been received on or after the 4th birthday. If the series given is a combination of OPV or IPV, 4 or more doses are required with the last dose being a booster on or after the 4th birthday)

  • Measles, Mumps, Rubella (2 doses of each with the 1st dose being given on or after the 1stbirthday and the 2nd dose no less than 4 weeks (28 days) later. Usually given as a MMR.

  • Hepatitis B (3 doses administered at recommended intervals) Interval between 1st and 2nddose must be at least 4 weeks (28 days), 2nd and 3rd at least 2 months (56 days), and 1stand 3rd at least 4 months (112 days)

  • Varicella (2 doses of varicella vaccine with the first dose being given on or after the 1stbirthday and the 2nd dose no less than 4 weeks (28 days) later, or statement from the physician verifying disease or laboratory evidence of varicella immunity

  • Meningococcal Conjugate – MCV4 (1 dose given on or after the 11th birthday)

Eye/Vision, Hearing, and Dental Examinations & Screenings

Dental Examination

  • Every student in grades Kindergarten, 2nd, and 6th must have a dental examination and submit proof of such an examination by May 15 of each school year.  The dental examination must have taken place within 18 months prior to May 15th of the school year.  Dental exams are a state requirement.

    Eye Examinations

  • Kindergarten students or students enrolling for the first time must have an eye exam by October 15th.  The eye exam must be completed within one year prior to the first day of the school year the student enters kindergarten or school the first time in Illinois.  A physician licensed to practice medicine in all of its branches or a licensed optometrist must perform the required eye examination.

    Vision Screenings

  • Vision screenings will be done annually, as mandated in the Illinois School Code, for children in the following programs/grade levels: Kindergarten, 2nd grade, 8th grade as well as students enrolled in our special education programs, those referred by teachers and students that are new to District 93. 

  • Parents are notified in writing when their child fails a vision screening. Vision screening is not a substitute for a complete eye and vision evaluation by an eye doctor. Your child is not required to undergo this vision screening if an optometrist or ophthalmologist has completed and signed a report form indicating that an examination has been administered within the previous 12 months and if that evaluation form is on file at the child’s school. Vision screening as part of a routine physical examination is not an option. If a vision examination report is not on file at the school, the child in the mandated program/grade level will be screened

    Hearing Screening

  • Hearing will be screened annually at grades Kindergarten, 1st grade, 2nd grade, and 3rd grade, as will all children in our special education programs, teacher referrals and students that are new to the district. Parents are notified in writing when their child fails a hearing screening.

  • Your child is not required to undergo this hearing screening if a physician and an audiologist have completed and signed a report form indicating that an ear examination and audiological evaluation have been completed within the previous 12 months and that form is on file at the school.

Transfer Students

A student transferring from out-of-state who does not have the required proof of immunizations by October 15 may attend classes only if he or she has proof that an appointment for the required vaccinations is scheduled with a party authorized to submit proof of the required vaccinations. If the required proof of vaccination is not submitted within 30 days after the student is permitted to attend classes, the student may no longer attend classes until proof of the vaccinations is properly submitted.
